Graduate Student Forms
There are a number of forms that you will be required to complete during your graduate career. Many of these forms are discussed below and can be printed from the web.
Mandatory Immunization and Health History Form
All students must entering the University of Florida must meet the mandatory immunization requirements and complete an immunization and health history. All individuals who have been accepted for graduate study should receive a yellow copy of this form.
Health Insurance
The GatorGradCare plan is available free of charge to students on assistantship or fellowship beginning January 2007. Students who want the GatorGradCare plan must enroll through the website below every school year.
UF Graduate School student forms
The UF Graduate School Web site has posted several forms often needed by students on the Web.
Form 2 (Degree Plan)
Form 2 is your degree plan and is submitted to the Dean of the College of Agriculture. The form requires a list of all courses the student intends to take during their graduate studies. This form must be approved and signed by all members of the supervisory committee. For a template of this form, click here.
